Individual Plans
All plans include an annual comprehensive wellness exam.
Family plan includes care for 2 adults and 2 children. Additional children are $10 each. There is a one-time registration fee of $30 per patient at the time of enrollment. Members enrolled on individual plans pay $20 per visit.

Business Plans
Family plan includes care for 2 adults and 2 children. Additional children are $10 each. In addition to routine medical care, on-site nutrition and fitness assessment are included in all Business Plans. Additional discounts for employee groups over 50.
